Maria Mora Pinzon, MD, MS, helped create a toolkit for health care providers, in partnership with the American College for Preventive Medicine (ACPM) and with funding from the Centers for Disease Control and Prevention (CDC).
The brain health resource provides a guide with illustrative examples to assist preventive medicine physicians and related partners to educate patients on Alzheimer’s disease and related dementias, reduce risk factors related to dementia, and improve the brain health and cognitive functions of populations by leveraging community-clinical linkages (CCLs). CCLs are collaborative efforts that connect health care systems with community-based organizations to improve health outcomes. Some of the common CCLs identified by the CDC include networking, collaborating and coordinating.
