Navigating challenges in the dementia journey with Memory Link

Wisconsin Dementia Resource Network invites you to a webinar. Join us to learn about Memory Link, an evidence-based resource navigation program to support people living with dementia and their care partners. The Resource Navigator’s ongoing, regular contact creates opportunities to assess unmet needs and assist the individual and their care partner create practical solutions and action steps. Assistance can include strategies to manage symptoms, facilitating transitions, connecting to relevant community resources, trying new communication techniques, or implementing a home safety change.

Presenters
Kristy Millar has 10+ years of working in Memory Care. She is a Certified Dementia Practitioner and Certified Alzheimer’s Disease and Dementia Care Trainer through the National Council of Certified Dementia Practitioners. Kristy has a passion for supporting and educating care partners and their loved ones affected by dementia.
Sarah Swant has extensive experience and empathy working with people with disabilities and their families, caregivers, and service providers, to facilitate and maintain safety, independence, and self-determination.
